A new diagnosis or chronic illness adds a considerable amount of stress in the life of the patient and family members.  Stress is oftentimes a barrier to a quick or successful recovery.  Horizon social workers are trained to assist patients and families at those critical times. 

Our social workers are available to listen and advise.  They are knowledgable of community resources that can be accessed and the requirements for eligiblity.  Social Workers are specialists at assisting patients to removal of their barriers to successful recovery.
