Horizon Home Care offers a comprehensive heart failure program for patients in all stages of heart disease.  Our nurses are trained to manage the cardiac patient in the home setting.  Our goal is to limit rehospitalization through skilled management of the disease process.  Through continual management of the patient at home we are able to decrease the number of acute episodes a patient will experience.

Our nurses train patients to manage their disease state through medication, diet, exercise, and life style.  Your Horizon nurse,  in conjunction with your physician, will work to assist you in achieving optimal recovery and longterm health.
