Horizon’s Supportive Home Care program offers services to keep people in their homes. Horizon offers Supportive Home Care in Milwaukee, Ozaukee, Waukesha, and Washington counties. Service may be a bath and house cleaning twice a week, round-the-clock care, or simply meals delivered daily. Each person requiring support in the home defines their level of care with our supportive home care team.

It begins with a professional evaluation of needs. Our registered nurse, with you will explore what options will best serve to meet your care requirements. You will pick from a variety of services in any combination and develop a plan for your care. Options available are:

• Meals delivered to you home
• Cleaning twice a week
• Someone to stay with you at night
• Transportation to doctor visits
• Meal preparation in your home daily
• Someone to care for you while family members are at work
• Round-the-Clock care
• Daily medication reminders
• Bathing every other day
