Horizon Home Care is a State-licensed, Joint Commission-accredited home health agency. The Agency is Medicare and Medicaid certified. Additionally, Horizon is a preferred provider for most health insurances that cover home health care. Horizon brings the latest techniques and technology available in the delivery for health care services to you - in your home.

Home care can be any combination of Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Work, and/or Home Health Aide services provided for a patient in their home.

All home care is provided under a physician's order. (The Agency will call the physician for the order.) In cooperation with the patient and physician, skilled home care professionals create a Plan of Care, setting achievable goals. Care is then delivered according to the Plan of Care created, until the reasonable achievement of those goals has occurred.
