Family Care is a case management unit funded by the Milwaukee County Department of Aging.
The employees in our Family Care program are Registered Nurses, Therapists, and Social Workers. Their responsibilities include assessment, authorizing release of funds, locating services and creating service plans.

Working in Supportive Home Care
The Community Services program at Horizon Home Care & Hospice includes Meals-On-Wheels and Supportive Home Care services.
Our Supportive Home Care program offers a wonderful opportunity for individuals who enjoy managing the home environment of our patients. Our Supportive Home Caretakers prepare meals, do laundry, run errands, and serve as a companions for our elderly patients. They do not provide personal care.
Meals-On-Wheels drivers are provided through our Volunteer Services. See the Volunteer Opportunities section.

Working in Hospice
Our hospice staff provides the in home, or some cases inpatient, care for patients who are living the last days of their life. Our nurses manage pain control and the physical status of the patient while our support staff work with the patient and their family to manage the grieving process. While many professionals think they could not handle the dying patients, our hospice staff feels privileged to provide the comfort measures and support that allow those last days of life to be meaningful.
End of life care is critical to the patient and their loved ones. At Horizon, we are proud of the care our Hospice staff provides. Our Hospice team manages our patients' pain, eases their fears and provides the therapies needed to make our terminal patients as comfortable as possible. Patients' loved ones are also supported through bereavement and chaplain services.
