Horizon is a private, not-for-profit organization sponsored by Froedtert Memorial Lutheran Hospital, Columbia St. Mary's and Community Memorial Hospital. We are licensed to provide home care and hospice services, are Medicare and Medicaid certified and accredited by The Joint Commission since June of 1999. The Joint Commission will conduct an on site survey some time during 2008 to re-accredit our agency.

We serve patients in southeastern Wisconsin, averaging a census of 1,000 patients served by a staff of more than 350 employees.

Horizon's Home Care goal is to promote the ability of patients to care for themselves to sustain life and health or recover from disease or injury. Our Hospice program focuses on optimizing quality of life and providing support for patients and those involved in their care.
